Oxtellar XR is an anti-seizure drug that comes as a tablet taken once daily. The recommended Oxtellar XR dosage will depend on how much you weigh, how old you are, how well your seizures are controlled, and other factors. Make sure to take each tablet on an empty stomach, at least one hour before or two hours after a meal.

An Introduction to Dosing With Oxtellar XR

The dosage of Oxtellar XR™ (oxcarbazepine extended-release) your healthcare provider recommends will vary, depending on a number of factors, including:
  • Your age
  • Your weight (in children)
  • How well your seizures are controlled
  • Other medications you are taking
  • Other medical conditions you may have.
As is always the case, do not adjust your dosage unless your healthcare provider specifically tells you to do so.

What Is the Recommended Oxtellar XR Dosage for Adults?

The usual recommended initial dosage of Oxtellar XR for controlling partial seizures in adults is 600 mg a day. After 1 week, your healthcare provider may slowly increase your dosage, as needed, by no more than 600 mg at a time. Your healthcare provider will wait at least a week between each dosage increase. 
Most people need to be treated with Oxtellar XR doses of 1200 mg to 2400 mg a day. While 2400 mg a day appeared to work better in clinical trials, this higher dosage was associated with more side effects.
People with severe kidney disease are usually started on Oxtellar XR 300 mg a day. The dosage may be increased slowly (no more often than once a week) by 300 mg to 450 mg, if necessary.
For older adults, the recommended starting dosage of Oxtellar XR is 300 mg or 450 mg a day. The amount may be increased slowly (no more often than once a week) by 300 mg to 450 mg, if necessary.
If you are taking certain other anti-seizure medicines, such as carbamazepine (Carbatrol®, Epitol®, Equetro®, Tegretol®), phenobarbital (Luminal®), or phenytoin (Dilantin®, Phenytek®), your healthcare provider may start you on Oxtellar XR 900 mg a day, and slowly increase your dose as needed from there. This is because certain other anti-seizure medicines can decrease blood levels of Oxtellar XR.
Oxtellar XR contains the same active ingredient as Trileptal® (oxcarbazepine), but in a long-acting form. If your healthcare provider is switching you from Trileptal, you may need higher total daily doses of Oxtellar XR.
